Collect Specimen and Prepare the CCF

Part of specimen collection process is to prepare the CCF (Custody and Control Form) and add the drug test record with some essential data such as the specimen number, laboratory, and collection date.

If you have configured the task to use a PDF (lab-specific or the nonspecific drug test form), the drug test record is added automatically when you save the form. These forms are designed to collect the information that populates the top portion of the Drug Test screen.

If you are not using a PDF, you can manually add the drug test record with as much information as possible in the top portion of the Drug Test data entry screen. A drug test record added during the specimen collection makes it easier to locate drug tests that have been collected but do not contain results. HL7 Drug Test Interface: the system can only import results to a drug test record with a matching collection date, specimen #, and test lab ID.

Using a drug test PDF form to add the drug test record. Skip to the next section if you are not using drug test PDF forms.

  1. Click the pencil icon in the F column on the Appointment Register to open the drug test PDF form.
  2. Complete the form and save.
    The drug test record is automatically added.

  1. Complete the fields in the top portion of the Drug Test data-entry screen and save.
    If this was a random test, use the look-up on the Random ID field to find the randomization for which this employee was selected.
  2. Record the Date CCF Received under the MRO section.
    • If the MRO for the test is on staff (has an S in the Staff field of the Medical Staff file), the Date CCF Received field automatically fills with the date of collection.
    • If the MRO is not on staff, this date field must be completed later with the actual date s/he receives copy two of the CCF.

Complete the collection and send the specimen

  1. Complete the CCF. Note: All DOT tests require manual completion of the five-part carbonless manifold form.
    • Manual process: Complete the paper CCF.
    • PDF form process: Complete the paper CCF.
  2. Attach the CCF to the specimen and send with the specimen.
