Problem List
The problem list provides a brief list of a patient's ongoing medical conditions at the time of a visit. It could also list prior surgeries or the results of major tests.

| Patient ID | The patient's unique identifier. The patient's full name is displayed adjacent to the ID. |
| Active Problem | Indicates the problem's status. When the box is checked, the status is Active. On an add record, this is checked by default. This is especially useful when excluding inactive records in search lists and reports. |
| Description | Describes the nature of the problem; free text field up to 80 characters. |
| Date Began | Enter the date the problem was first discovered, if known. |
| Date Ended | Enter the date the problem no longer existed, if known. |
| ICD | ICD code pertaining to the problem with the description in the adjacent field. This links to . |
