1500 Claim Form Standard insurance claim form (formerly HCFA-1500) from the Centers for Medicare and Medicaid Services (CMS).
ADA Americans with Disabilities Act, a comprehensive Civil Rights law to assist people with disabilities; enforced by Department of Justice.
ADL Activities of daily living.
BAT Breath Alcohol Test or Breath Alcohol Technician, the licensed person who administers the breath alcohol test.
Billing Jump An operation that occurs when the system opens the Invoice Find screen after you select an order and click Invoice. For more information, see the Billing Module section in Program Preferences.
BWC Bureau of Workers' Compensation.
C/A/N/T Appointment status options on Appointment Register screen and Treatment screen: Canceled, Appointment, No-show, Treatment.
CCR Continuity of Care Record
CDC Centers for Disease Control and Prevention, part of DHHS (see DHHS).
Chart An attached PDF form that has been opened and saved (with or without data entry) and any other type of file that has been added to a patient's order via the Attach File option.
CLIA Clinical Laboratory Improvement Amendments, a federal agency under CMS.
CM Case Manager, Case Management; a system of monitoring, guiding, and supporting an injured worker as s/he recovers from a workplace accident/illness.
CMS Centers for Medicare and Medicaid Services, a federal agency under DHHS.
COW Computer On Wheels; mobile computer workstations for healthcare workers.
CPT Current Procedural Terminology; a system for coding medical services and procedures performed by physicians; required for most physician billing.
CQI Continuous Quality Improvement, a method to ensure quality of services.
DEA Drug Enforcement Agency, a federal agency.
DER Designated Employer Representative, an individual identified by an employer as able to receive communications and drug or alcohol test results directly from MROs, BATs, screening test technicians, collectors, and substance abuse professionals.
DHHS Department of Health and Human Services, a federal agency.
DOL Department of Labor, a federal agency.
DOT Department of Transportation, a federal agency.
D, P, I Order status on the Orders screen: Data (D), Print (P), Invoice (I).
EAP Employee Assistance Program, an employer-sponsored program to assist employees experiencing difficulties that impact their work.
EBT Evidential Breath Testing; the test and/or device used in breath alcohol screening.
EDI Electronic Data Interchange.
EFT Electronic Funds Transfer.
EHR Electronic Health Record, the aggregate electronic record of health-related information on an individual that is created and gathered cumulatively across more than one health organization and is managed and consulted by licensed clinicians and staff involved in the individual's health and care (per National Alliance for Health Information Technology) (see also EMR).
EMR Electronic Medical Record, the electronic record of health-related information on an individual that is created, gathered, managed, and consulted by licensed clinicians and staff from a single organization who are involved in the individual's health and care (per NAHIT) (see also EHR).
Entry Date The actual date that a billable item is entered in SYSTOC. This is controlled by the program and cannot be edited by the user. The Entry Date and the Service Date often differ. It is important to use reports based on Entry Date when trying to balance accounting.
EOMB Explanation of Medical Benefit, a summary from an insurer that records the reason(s) an invoice is rejected/adjusted or clarifies a payment made.
FDF PDF Form Data Format. Form data can be imported and exported using the SYSTOC FDF library. Custom legacy tap2chart templates can be imported into Tap2Chart using the FDF template conversion tool.
FEF Forced Expiratory Flow, part of a pulmonary function screening.
FEIN Federal Employer Identification Number, tax identification provided to a company by the IRS.
Flattening Flattening a PDF form reduces the file size, permanently prevents additional data changes, and secures the document.
Form A blank PDF.
FROI First Report of Injury.
FVC Forced Vital Capacity, part of a pulmonary function screening.
HCFA Health Care Financing Administration, now CMS (see CMS).
HCFA-1500 Obsolete term once used for a standard insurance claim form, now known as the 1500 Claim Form (see HCFA).
HCPCS Healthcare Common Procedure Coding System, a standardized coding system used to process claims to Medicare and other health insurance programs. HCPCS is divided into level I for medical services and procedures (CPT codes), and level II for products, supplies, and services not included in CPT codes. Level II codes are alphanumeric codes, consisting of a single alphabetical letter followed by four numeric digits; CPT codes consist of five numeric digits.
HIPAA Health Insurance Portability and Accountability Act, a federal law protecting health insurance coverage and the security and privacy of health data.
HMO Health Maintenance Organization, a system of insuring, monitoring, and managing healthcare; primary goal is to provide efficient healthcare at lower costs.
HPI History of Present Illness, a common medical abbreviation.
HRA Health Risk Analysis, a tool for patient health and wellness education.
HX History, a common medical abbreviation.
ICC Interstate Commerce Commission, a federal agency.
ICD International Classification of Diseases; provides classification and comparison of statistics on a world-wide basis.
IME Independent Medical Examination; used in injury cases when worker's condition and claim information need an unbiased evaluation.
ImpRenew Licensing program for the generic import program.
iSYSTOC A free web-based service that allows authorized client companies, insurance carriers, and/or off-site physicians to view patient data generated by SYSTOC over a secure, encrypted connection.
JCAHO Joint Commission on Accreditation of Healthcare Organizations, a commission that provides accreditation and related services to support performance improvement in healthcare organizations.
LTG Long-term goals.
MCO Managed Care Organization, a system of healthcare delivery that influences utilization and cost of services and measures performance.
METS A measure of the intensity of effort, expressed as the ratio of work metabolic rate to resting metabolic rate.
MMI Maximum Medical Improvement; reference to the best health condition an injured worker can attain, with the recognition that full recovery is not likely.
MRO Medical Review Officer, a medical doctor who interprets urinalysis test results reported by a lab to ensure a scientifically valid result and who determines whether a legitimate medical explanation could account for any confirmed positive results.
NAICS North American Industry Classification System, jointly developed by the U.S., Canada, and Mexico to provide new comparability in statistics about business activity across North America.
NCCI National Council on Compensation Insurance; maintains a nation-wide database of detailed statistical information on workers' compensation cases.
NEC Not Elsewhere Classified (commonly referred to as Not Otherwise Classified). This terminology is often found in the descriptions of ICD codes or NCCI codes.
NIOSH National Institute for Occupational Safety and Health, the federal agency responsible for conducting research and making recommendations for the prevention of work-related injury and illness; part of CDC (see CDC).
NKA No Known Allergies, a common medical abbreviation.
NOS Not Otherwise Specified. This terminology is often found in the descriptions of ICD-9 codes or NCCI codes.
OSHA Occupational Safety and Health Administration, a federal agency that is part of the Department of Labor.
PCP Primary Care Physician.
PDF Portable Document Format, a universal file format that captures all the elements of a printed document as an electronic image that can be viewed, navigated, and printed. Interactive forms containing data fields to collect and manage data are often created in PDF file format.
PDF Editor A program used to create and manipulate PDF documents containing interactive fields and similar types of controls. These programs are not typically free. Adobe Acrobat Professional, Foxit Phantom, and CutePDF Professional are examples.
PDF Printer A free print driver used to create a PDF document from any document that can be printed. Instead of printing to a physical printer, printing is directed to the print driver and the user is prompted for a file name to assign to the PDF file that is created. Common examples are Adobe PDF printer, PDFCreator, and CutePDF printer. PDFs within SYSTOC can be printed using the SYSTOC PDF reader.
PDF Reader The SYSTOC PDF reader is used to view PDF documents.
PFT Pulmonary Function Test, a respiratory screening.
PHI Protected Health Information.
POC Plan of Care.
POR Provider of Record.
PPD Purified Protein Derivative, the Mantoux tuberculosis skin test.
PPO Preferred Provider Organization, an arrangement between a group of providers and an employer or other group to offer price discounts on services in exchange for access to a higher volume of patients.
PRONA Process Report of New Activity, a report used when processing bills.
PTA Physical Therapy Assistant.
Report Tool Report Importer and Reports - Forms Updater.
ROM Range of Motion.
RVU Relative Value Unit, stores any one of several coding methods used to quantify resources needed to provide a product/service; useful for statistical purposes. In SYSTOC, the RVU is stored in the Fee table.
Service Date The date that a billable service occurs or an item is dispensed. The Service Date is often different from the Entry Date. Service Date should be used for reports that analyze activity, never for reports that balance accounting.
SIC Standard Industrial Code, used by the Bureau of Labor Statistics to compile economic and labor market data; replaced by NAICS (see NAICS).
SOAP Subjective, Objective, Assessment, and Plan; a standard format for medical transcription.
SSN Social Security Number.
STG Short-term goals.
STS Standard Threshold Shift; in an audiological exam, a change of at least 10 decibels constitutes a shift per OSHA.
SystocImporter Generic import program.
Tablet PC Portable computer that can be used as either a laptop or a digital notepad. As a notepad, the orientation of the screen rotates 180 degrees and closes to cover the keyboard. While in notepad orientation, a stylus is used to write or make selections directly on the screen.
TIN Tax Identification Number.
TOS Type of Service.
TPA Third Party Administrator, a person who processes claims and offers administrative services pursuant to a service contract; does not include an insurance carrier or employer sponsoring a WC plan.
TPW Times per week.
UB Claim Form Uniform Billing claim form (formerly UB92); also referred to as the HCFA 1450.
UPIN Unique Physician Identification Number, an identification system for healthcare providers.
WCB Workers' Compensation Board.
WFL Within Functional Limits.
