Line Item UB Fields
The Line Item UB screen contains the Standard Line Item fields, some of the 1500 Claim Form (HCFA) fields and a few that do not appear on the other invoice forms.

| Revenue Code | Four-digit code corresponding to a description or abbreviation that identifies a specific accommodation and/or ancillary service. Since the standard set of codes supplied by 1500 Claim Form (HCFA) contain only three digits, a leading zero is added for electronic transactions. |
| TOS ID | Type of Service code from the Treatment screen. |
