Vision

Vision testing is required for a number of physical examinations, such as those done for the Interstate Commerce Commission (ICC). Vision test results may be recorded in SYSTOC via the Vision data-entry screen or with a PDF form.

The Vision Results report provides a succinct summary of each test and may be specified to include a descriptive sheet to explain the results to the patient (set Print Description? = Y).

Label Description

Patient ID

Patient's identification number.

Date

Date of the examination.

Staff ID

ID of person performing the test.

Loc ID

Location ID for multi-site clinics.

Company ID

Company ID of employer at time of test.

Dept ID

Employee's department at the time of the test.

Job ID

Employee's Job ID at the time of the test.

Uncorrected

Test results without eyeglasses, for near and far vision.

Near/Far Vision

Results for left eye, right eye, and both.

Corrected

Test results with eyeglasses, for near and far vision.

Near/Far Vision

Results for left eye, right eye, and both.

Peripheral Vision Records the lateral extent of a patient's vision.

Nasal/Lateral

Two measurements for each eye that define the patient's arc of vision, measured from the nose to the outer periphery. Choose N (Nasal, no peripheral vision), 55, 70, or 85 from the drop-down list.

Color Discrimination

Enter Y for acceptable, N for unacceptable. Describe in the unlabeled adjacent field.

Depth Perception

Enter Y for acceptable, N for unacceptable. Describe in the unlabeled adjacent field.

Fusion

Indicates potential imbalance in eye-positioning muscles. Enter Y for acceptable results, N for unacceptable. There is no individual comment field.
