Hepatitis B

Use the Hepatitis B or the appropriate Hepatitis B PDF form to enter all activity related to the hepatitis B immunization series, creating one record for each injection in the series. Information entered in the hepatitis record is automatically copied to the patient’s Immunization record (also accessed from the Screening Folder).

If the appropriate recall series has been created, and data entry for the first injection is performed through Orders (and not directly in the Hepatitis B screen accessed from the Screening Folder), SYSTOC will generate automatic recall records for reminding a patient to return to the clinic for subsequent injections in the series. Label Description
Patient ID Patient’s identification number.
Due Date vaccination is due.
Done Date vaccination is given.
Test ID ID code for the vaccination (normally HEPB1, HEPB2, or HEPB3). Use HEPBDEC if the person refuses to have the immunization, HEPBPREV if the patient can prove he or she was previously immunized elsewhere.
Results Comment Use this field to enter other data about the immunization, if desired.
Staff ID Medical staff person administering the immunization.
Injection Site Body location where injection was given.
Mfr/Lot # Vaccine lot number.
Expiration Date Expiration date for vaccine.
Company ID Company ID at time immunization.
Dept ID Department ID at time of immunization.
Job ID Job ID at time of immunization.
Location ID Multi-site location code.
Injury ID Appropriate injury ID number if activity is related to an injury.
