F3 Memo on the Treatment Screen
The F3 Memo attached to the treatment table is the medical record that is included with HCFA-CMS 1500 invoices. The F3 Memo only captures information about a specific visit. You can type text directly into the memo or use a template that you have created beforehand. The template can be very simple, such as SOAP note headings; or it might include statements that can be checked off.

The Injury Management Report prints all the treatment memos for a patient. Changes to the F3 Memo are compiled in a SYSTOC database as a permanent record of the memo. You can access the record of changes via the Treatment Memo History report. If data entry for an injury is done via the Tap2Chart form, the narrative that is produced on this form will be written to the Treatment F3 memo.
