Rehabilitation Claim

The Claim section of the screen contains details about the insurance claim for this case. Careful attention to this section can help to ensure prompt payment for care and prevent the clinic from providing services that are not covered. Most fields here are also in the Injury Claim table and changes here write back to that table.

Label	Description
Plan Begin Date	Date the medical management of the claim began.
Claim Assignment	The name of the benefit organization's representative handling this claim.
Active Code	Indicate `Y` or `N`, is this claim active?
Act/Inact Date	Indicate the date the claim became active or inactive, whichever is the most recent status.
Claim Status Code	Choose from the drop-down list of status codes. As the cursor hovers on each code, the meaning is explained at the top of the screen.
Claim Status Date	Date the most recently-entered Status Code became effective.
Deductible	The deductible for the insurance plan, in dollars and cents.
CoPay	The co-payment amount, in dollars and cents.
Coverage%	The percentage of the claim payable by the insurer.
Benefit Limit	Indicate any benefit limit, expressed as dollars per year, visits per year, consecutive days, etc.
Date Limit Reached	If the limit has been reached, indicate the date.
