Allergies and Conditions

Use this choice to record the patient's chronic conditions or allergies. Use the memo for any clarifications, if necessary. The first several lines of the memo appear on the data screen.

The following information that is part of the JCAHO requirements for any patient receiving ambulatory services over time should be recorded in the allergies/conditions module:
  • Known significant medical condition and diagnosis
  • Known significant operative and invasive procedures
  • Known adverse and allergic drug reactions

Allergies/Conditions screen

Label Description
Patient ID The patient's unique identifier. The patient's full name is displayed adjacent to the ID.
Date Diagnosed Enter the date the allergy or chronic condition was diagnosed, if known.
Condition ID Code identifying an allergy or chronic condition suffered by the patient with the description of the allergy/condition in the adjacent field. This links to File Maintenance > Setup - Orders > Task Prototypes.
Memo The memo is accessed from a button on the data form or use Alt+M, rather than F3. The first few lines of information typed in this memo are always displayed on the form, which helps ensure that vital information is not missed.
